Simple step-by-step picture guide for creating your ABHA health ID, checking PMJAY eligibility, and using Swasthya Sathi — even if you are not tech-savvy.
Your unique 14-digit digital health ID — free & takes 5 minutes
Open Google Play Store → Search "ABHA" → Find the app by National Health Authority → Tap Install.
After installing, open the app. Tap Create ABHA. (If you already have ABHA, tap Login instead.)
Select Using Aadhaar (recommended — faster, more features) or Using Mobile Number. Both are free.
Type your 12-digit Aadhaar number. Tick the consent checkbox. Tap Send OTP. OTP arrives on your Aadhaar-linked mobile within 30 seconds.
Enter the 6-digit OTP you received. Tap Verify OTP. Your name and address appear automatically from Aadhaar.
Choose your ABHA address (like yourname@abdm). Use the suggested one or type your own. Tap Create.
Your 14-digit ABHA number is created. Tap Download ABHA Card to save as PDF. Show QR code to any doctor.
Find out if your family gets ₹5 lakh free hospital cover
Open your browser. Go to beneficiary.nha.gov.in. Tap Am I Eligible? button on the homepage.
Enter your 10-digit mobile number. Type the captcha letters shown. Tap Generate OTP.
You get a 6-digit OTP on your mobile. Enter it in the boxes. Tap Verify OTP.
If eligible, green ELIGIBLE screen shows your PMJAY ID and ₹5 lakh coverage. Take a screenshot!
Tap Download e-Card. It saves as PDF. Show it at any empanelled hospital for free treatment — no physical card needed.
₹5 lakh free cover — every WB family is eligible, no income limit
Collect: Voter ID (WB address), Aadhaar of all family members, 2 passport photos of woman head of family, active mobile number.
Visit your nearest Gram Panchayat Office (village) or Ward/Municipality Office (town/city). Ask for the Swasthya Sathi enrollment desk.
Officer gives you a form. Fill in all family member details — name, relation, Aadhaar, date of birth. Write in CAPITAL LETTERS. Officer may help you.
All adult family members give their fingerprint on the scanner. This links Aadhaar to the account. Takes 1–2 minutes per person.
You get a printed slip with a reference number. Keep this slip safely — you need it to collect your card in 2–4 weeks.
When ready, collect from Gram Panchayat. Your Swasthya Sathi Smart Card has family photo, QR code, and ₹5 lakh cover. Show at any empanelled hospital.
eSanjeevani — Government of India, 7 AM–11 PM, 7 days a week
Go to esanjeevani.mohfw.gov.in or download eSanjeevani OPD app from Play Store. Tap New Registration if you are new.
Enter name, mobile, age and gender. Tap Send OTP. Enter OTP to create your account. You only register once.
Select your state (West Bengal). Choose doctor type — General Medicine, Child Health, Mental Health, Skin, etc. Tap Book Token.
You get a token number. Wait on the page. When your turn comes, Start Consultation button appears. Tap it. Camera starts — doctor joins the call.
After the call, doctor types your prescription. You receive it on SMS and can download from the app. Upload to Deinrim Health records.
